I have made a voluntary request to participate in ACTIVITY at SAVY International Inc which includes but not limited to sessions of yoga, pranayama, meditation, kriyas, marma, mudras, nada, swara and other constituents of yoga lifestyle. I recognize that these require physical exertion that may be strenuous and, even with clear instruction and precautions, there is a possibility of a physical injury, and I am fully aware of the risks and hazards involved. I affirm that it is my responsibility to advise every instructor at the yoga studio before every session, of any conditions which may limit my participation in the ACTIVITY. I understand that ACTIVITY is not a substitute for medical attention, examination, diagnosis, or treatment. I understand that it is my responsibility to consult a physician prior to and regarding my participation. If I have consulted a physician, I have taken the physician’s advice. I have no psychological, medical, or emotional condition, which would prevent me from taking part in the ACTIVITY. I assume full responsibility for any risk or injury I may sustain as a result of my participation. I will not perform any postures to the extent of strain or pain. I accept that neither the instructor, nor the hosting facility, is liable for any injury, or damages, to person or property, resulting from the attending of the ACTIVITY. I confirm that I do not have a taboo of touch and I agree to being corrected verbally and physically during the course of instruction. I also affirm that I shall be committed to attend the program as per schedule and it will be my sole responsibility to attend the program without any break. Any missed classes during the programs will not be compensated in any way. I also understand that the role of the instructor assigned will be only advisory and no results whatsoever are guaranteed. I also confirm that I have not been assured of or given any warranty or guaranty of any miraculous cures with any of the ACTIVITY. I understand that I have been permitted to use the contents of the training only for myself. I agree and confirm that I would not record, teach, share or divulge any kind of information about the program or its contents, without prior written permission from SAVY International Inc; any activity contrary to this would amount to breach of trust and an infringement of the legal rights. I am responsible for all personal belongings. Yoga studio will not be responsible for lost or stolen belongings. In accordance with Canadian Anti-Spam Legislation I give my consent to receive electronic communications from SAVY International Inc regarding upcoming workshops, training, announcements, invitations, events, promotions and all other related electronic communications. I understand that services are being provided subject to my undertaking that in case of any controversy whatsoever, I shall inform SAVY International Inc in writing in order to seek a resolution from them. SAVY International Inc. reserves the right to refuse access and to ban patrons from the facilities if other members or staff experience any form of harassment. I hereby agree to irrevocably release and waive any claims that I have now, or hereafter may have, against SAVY International Inc., its teachers, instructors, owners, directors, officers, trainees, sponsors, and/ or other participants in events, activities or workshops. I, my heirs and legal representatives, forever release, waive, discharge and covenant not to make any claim against, sue or attach the property of SAVY International Inc., or any of its affiliated organization or above-mentioned parties for any injury or death caused by negligence or other acts. I have read the SAVY International Inc. Policy Statement and understand the terms and conditions stated and voluntarily agree to them.

I give my consent to use of my photographs/ recording of videos of practice sessions for academic purposes by SAVY International Inc. and/or any of its assigned trainees/ instructors.
